Vision Plan Options:
USI Affinity has worked with Davis Vision to develop four comprehensive vision options that are available exclusively to ACBA members.
- Allows members to get an eye exam as well as frames, lenses and contacts once every 12 or 24 months at a participating provider.
- Members can get specialty lenses and frames at a co-pay.
- Level and the plan includes a generous reimbursement schedule for any out-of-network products or services.
- Members can access the majority of the benefits once every 12 months.
